Not too sure what the issue could be since it looks like you're on chrome as well. Also seems to work fine in other browsers.

Are you running any other MAL userscripts?

I run quite a few like https://i.gyazo.com/9ed7990cdbbf0a23a44d1cf932af3cd2.png so maybe? I've tried reinstall it many times and it never works but I do remember my first install it downloaded some .js file for some reason and got popup error which I can't quite remember what it said from chrome.

It shouldn't be downloading any kind of .JS files, since the script itself is rather barebones.

Only thing I can really suggest is toggling off some userscripts and see what one is causing conflicts.

Disabled every active userscript on MAL and still no change and then I even tried it on another PC and it still doesn't work, I really wonder if I'm alone in this of those that have installed it.

I've just pushed out an update (1.0.4) which should resolve your issue.

Problem was Chrome 48 and below doesn't have support for deconstructing assignment (https://developer.mozilla.org/en-US/docs/Web/JavaScript/Reference/Operators/Destructuring_assignment).
I'm using Chrome Dev which runs Chrome 49 & has support for it, while Chrome Stable runs Chrome 48 and lacks the support.
