Firefox works fine for me with Tempermonkey.
With Greasemonkey, I did not see the extra menu items, as well.
Then you have some conflicting scripts or IDK what. I made this script on Firefox (92.0, 64-bit) + Greasemonkey (4.11) so I know it works fine.

Installed latest version, restarted firefox, but still can't see the extra menu items.
It used to work a few days ago.
