YouTube is currently rolling out massive UI changes, which they do through "experiments" (I think that's the name). Basically, different people get different changes to their UI, to speed up the feedback process and see how users interact and respond to different changes.
Depending on what experiment they're serving your browser, this script may be broken. I've seen several iterations of the new UI experiments in the last few weeks but have not experienced any that actually break the script, so there's nothing I can do at the moment. The script may begin working on its own for you in the future.
