Hi Swissy, I haven't thought about this script for like 7 years. Hopefully someone else can copy it and update it for you. You can try to recruit someone on the requests forum:

I've been using this userscript since the time it was released-- it's one of the best, simplest scripts, ever.
I do think it does still work (unless one of my dozens-of-other installed scripts is doing the same thing): it seems to be a timing issue - MAYBE, if the page loads too fast, then this script doesn't fire. First, try putting it at/near the top of your lists of installed scripts; if that does work, try at or near the bottom.
The &list &index crap bogs it down with the playlist interface.
@Swissy: using a content blocker that has operator '$removeparam', try adding to your custom filters list:
||www.youtube.com^$removeparam=list
