Settings button should appear always, but it can take some time :(

The 640x360 style does not work correctly (currently version 0.4 but it's always been this way). Although the video area is smaller, the video itself does not change size, and the controls on the lower right are also hidden.
Additionally when this is fixed, I would really prefer a choice of sizes because my preferred size is 853.333 x 480.
Also wanted to let you know the settings button sometimes doesn't appear; I haven't figured out why yet.
