UVa CCU login helper

Adds form wrapper and field names to login screen so that Firefox password manager (along with Secure Login and Saved Password Editor) can detect it.

Allows Firefox's password manager to detect a login field for the new (as of June 2014) University of Virginia Community Credit Union website. This is accomplished by annotating the form fields with name attributes, removing autocomplete=off, and wrapping them in a form element.
